This week, the Trump administration announcedthe funding levels for its newly launched Rural Health Transformation Program, but uncertainties persist regarding which healthcare providers will receive the money and how successful the initiative will be in enhancing rural health care quality and availability.

The most challenging aspect of rolling out the Rural Health Transformation Program isn't about distributing funds to states," said Harold Miller, president and CEO of the Center for Healthcare Quality and Payment Reform in Pittsburgh, in an email. "It's making sure the money truly maintains and enhances rural healthcare services.

Over the last five years, more than 100 rural hospitals have stopped providing childbirth services, over 40 have shut down their inpatient departments, and more than two dozen have completely closed," Miller stated. "If such closures keep happening in 2026, even with $10 billion in new funding available, the Rural Health Transformation Program would be considered a failure.

Award Amounts Vary Considerably

Under the $50 billion initiative, included in the budget reconciliation bill signed by President Trump on July 4, states will receive initial funding this year from CMS, with average awards of $200 million, ranging from $147 million to $281 million. Half of the funds will be distributed equally among the states, while the remaining half will be assigned based on several criteria, such as the level of rurality in each state and the policies being considered or enacted to improve access to rural healthcare, according to CMS.noted in a press statement on Monday.

Over 60 million Americans residing in rural regions are entitled to equal access to high-quality care," stated HHS Secretary Robert F. Kennedy, Jr. in the statement. "This landmark funding empowers local hospitals, clinics, and medical professionals to manage their communities' health services... residents in rural areas will now receive cost-effective healthcare near their homes, without facing administrative barriers.

Lori Tremmel Freeman, the CEO of the National Association of County and City Health Officials, stated that the funding, although appreciated, will not even begin to compensate for the reductions in public health financing that the administration has already implemented and those that are expected to be included in the reconciliation bill. "There's no way this initiative can possibly offset the damage that has already taken place, particularly on the public health front, for the well-being of our nation," she mentioned during a phone conversation.

According to an analysis by KFFA nonprofit health information organization states that rural areas will experience a reduction of $137 billion in Medicaid funding over the next decade due to other parts of the bill, which is also referred to as the One Big Beautiful Bill. Because of these expected reductions, "there are many people watching this [grant] money closely," Freeman noted.

How Will the Funds Be Allocated?

As per CMS, the money will be allocated for:

• Enhance preventative, initial, maternal, and psychological health services and establish additional entry points that make healthcare more accessible near residential areas.
• Promote training for healthcare professionals, residency programs, hiring initiatives, and retention strategies, as well as innovative routes that allow students to start their careers in their local areas.
• Upgrade rural infrastructure and machinery; enhance cyber protection and compatibility; and increase telehealth services, remote health tracking, and digital solutions that support prompt access to medical care.
• Create focused hub-and-spoke frameworks, rural regional centers of excellence, integrated data-sharing systems, and rural clinical networks.
• Evaluate innovative primary care and outcome-focused care approaches, enhance cooperation between rural and other healthcare providers, and encourage teamwork across regions.

The American Academy of Physician Associates (AAPA) commended the allocation of the grants, highlighting that CMS "integrated AAPA's state practice environment rating scale into its evaluation process, making updated physician associate practice laws a key consideration in determining the funding amounts for states."

"Healthcare in these under-resourced areas cannot fully advance as long as there are existing laws that limit [physician associates'] capacity to practice at the peak of their license, which means to the full extent of their education, training, experience, and skills," said Chantell Taylor, Chief of Advocacy and Public Affairs at AAPA.noted in a press statementWhich is why the [grant program's] motivations to update practice laws and promote additional workforce changes are crucial for addressing current access and workforce issues.

Rural Versus Urban Providers?

A major issue that still needs to be addressed is which providers and organizations will receive the grant money in each state. "There is a significant risk that a large portion of the funding for services will go to big urban hospitals, instead of small rural hospitals and physician offices, especially if states or CMS impose heavy [paperwork] requirements on those who receive the funds," Miller stated. "The services are meant to help people in rural areas, but the service providers don't necessarily have to be based there. This is beneficial for allowing things like telehealth with specialists, but it's not restricted to that. Every academic and tertiary care hospital can argue that it serves rural residents because some individuals from rural areas visit these facilities."

Furthermore, he stated, "CMS is permitting as much as 10% of the funds to be allocated to new companies for testing experimental technologies. While there are numerous ways technology can enhance healthcare access for people in rural areas, a mobile application cannot deliver a baby, collect blood, sew a wound, or perform a CT scan. These are the services that rural residents require if they are pregnant, have diabetes, or are injured, and they risk losing access to these services unless more action is taken to stop the closure of rural healthcare providers."

He also mentioned that no funds may be allocated for raising payments for current services — such as physician services — however, the money can be utilized to cover new services or to provide care for patients without insurance.

The administration also revealed the establishment of the Office of Rural Health Transformation, which aims to "collaborate with HHS agencies, states, healthcare facilities, health advocacy organizations, and other important partners to provide effective guidance and program efforts that enhance rural residents' access to medical services," according to the agency.Federal Register notice.

However, Freeman has some reservations regarding how this office will function, as while the CDC has been the primary federal entity collaborating with state and local health departments, "rural health has always been part of the larger [scope] within HHS... I'm concerned about the ongoing separation between the broader agency that oversees rural health and [the CDC]; we'll have to observe how HHS's new group interacts with CDC's rural health division. I hope there is a strong, close relationship."

Miller stated that since many of the reductions in the reconciliation bill will not take effect until 2027 or 2028, "the real effect on rural providers will rely on how states carry out the changes ... In 2026, the main focus for both states and the Office of Rural Health Transformation should be making sure that small rural providers can keep offering services to their communities."
